Vaccine Waivers and Exemptions

Hepatitis B Vaccine Waiver

On-campus students are required to have at least one dose of Hepatitis B, present lab evidence of immunity, or sign the waiver to the decline the vaccine. These are the steps to decline, or waive, the vaccine:

  • Read the CDC’s Vaccine Information Statement
  • Check the box underneath the Hepatitis B vaccine section of the UF Immunization Form
  • Sign and date the waiver (You must provide a legal signature to waive the vaccine. Please do not type your name.)

Meningitis ACWY Vaccine Waiver

On-campus students are required to have one dose of Meningitis ACWY at 16 years old or sign the waiver to decline the vaccine. These are the steps to decline, or waive, the vaccine:

  • Read the CDC’s Vaccine Information Statement
  • Check the box underneath the Meningitis ACWY vaccine section of the UF Immunization Form
  • Sign and date the waiver (You must provide a legal signature to waive the vaccine. Please do not type your name.)

Medical Exemption

Temporary or permanent medical exemption to vaccination will be granted after receipt of documentation. If a vaccine is contraindicated, have your doctor provide a note on letterhead stating the reason that you cannot receive the vaccine.

Personal Tenets or Religious Exemption

Students must provide a copy of their K-12 immunization exemption record. They will then be asked to sign the UF Exemption Form to transfer the K-12 exemption to higher education.
