Vaccine Waivers and Exemptions

Vaccine Waivers

Some vaccine requirements are met with either proof of vaccination OR a signed declination from student and/or parent/guardian if the student will be under 18 at the start of classes. Students in non-clinical health programs can choose to waive the Hepatitis B and Meningitis MCV4 vaccines. Students in clinical health programs may waive the Meningitis MCV4 vaccine.

Hepatitis B Vaccine Waiver

If you are not vaccinated and you wish to decline the vaccine, please follow these steps.

  1. Read the CDC’s Vaccine Information Statement
  2. Check the box underneath the Hepatitis B vaccine section of the UF Immunization Form
  3. Sign and date the waiver (image provided below).

PLEASE NOTE: You must provide a legal signature to waive the vaccine. Please do not type your name.

Meningitis MCV4 Vaccine Waiver

If you are not vaccinated and you wish to decline the vaccine, please follow these steps.

Exemptions

PLEASE NOTE: If a vaccine-preventable disease occurs on campus, students who have not been vaccinated or do not have immunity verified by lab test may be excluded from attending classes or other campus activities.

Age

If the student was born before January 1, 1957, an age exemption applies for measles (rubeola), mumps and rubella (German Measles) only.

Beliefs/Personal Tenets

To discuss a Beliefs/Personal Tenets Exemption, please call the Health Compliance Office at 352-294-2925.

Medical

A formal Medical Exemption may be completed for persons with certain medical conditions and/or contraindications (permanent or temporary) to certain vaccines. A letter from the student’s physician or the official State of Florida Medical Exemption form must be submitted to the SHCC. Additionally,  it is recommended that the student also do a baseline measles, mumps and rubella antibody lab test. If the student cannot complete the exemption form during Preview/orientation, call Health Compliance at (352) 294-2925.
