First, get your immunization records ready.
Second, submit your records on our portal.
Please allow at least 1-2 weeks for your registration hold to clear after submitting your records. Review our page for video instructions for more guidance.
Viewing or Retrieving Your Immunization History
Are you a Florida Resident or did you receive vaccines in Florida?: Please allow at least 1 week from your UF acceptance/account creation for any Florida SHOTS records to pre-populate.
- Log into the Student Self-Service Portal
- Go to the Immunizations section.
- Click “View Missing Immunizations” to check if all of your required vaccines are already on file. If they are, you’re good to go!
- Check your One.UF account to see if your immunization hold has been cleared. Missing vaccines will appear on your To-Do Items.
Newly admitted out-of-state domestic students: Use our portal to request your out-of-state immunization records.
- Log into the Student Self-Service Portal .
- Go to the Immunizations section and click the blue icon to request your records.
- Specify the state(s) in which you have received vaccinations.
- Complete the information for each state including your name and associated address.
- Click “Finish & Complete” and agree to the terms to access your records.
- Check back on your Immunizations page after a few hours for updates.
Submitting New Records
If you have vaccines that are not listed in a state registry, you’ll need to fill out a specific immunization form and follow these instructions:
- Have a doctor’s office, clinic, or health department fill out the required UF form.
- Record must be signed, dated, and stamped by health care provider or office representative.
- Any corrections to the record must be initialed and dated by the health care provider.
- The document must be legible and in English. Illegible documents will not be processed.
- Keep a copy for your records.
- Next steps: Log into the Student Self-Service Portal and go to the Immunizations page .
- Click on each dropdown menu to view options.
- If you have any of the menu options on your vaccine record, select the item to enter your dates. The entered dates will be shown on the right side of the screen.
- Repeat step 4 for all menus.
- After entering all dates, click “Submit”.
- Review the dates on the confirmation page.
- Upload the immunization form and any lab reports. You must upload your records in order for the entered dates to be saved. Data entry and form upload must be completed in the same session.
- You will see a confirmation window of successful upload.
- Allow about a week for verification.
- Check your One.UF account to see if your immunization hold has been cleared. Missing vaccines will appear on your To-Do Items list.

Veterinary Medicine Form -completed upon admission into the College of Veterinary Medicine
Minor Treatment Consent Form-for students who will be under 18 at the start of classes
