Why does UF require proof of immunizations?
The University of Florida is dedicated to preserving the health and wellness of our campus. Vaccine-preventable diseases do still exist and contracting these illnesses can have an adverse effect on a student’s health, well-being and ability to reach optimal academic performance. The most effective way to preserve and protect our campus from outbreaks of these infections is by establishing an immunization requirement. Therefore, University of Florida students shall be required to show documentation of specific vaccinations or proof of immunity as set forth in Florida statutes 1006.69, Florida Board of Governors Regulations Chapter 6 as well as recommended by the Advisory Committee on Immunization Practices (CDC).
Submit Immunization Records
I am a Florida resident and/or received vaccines in Florida:
If you are a Florida resident and/or received vaccines in Florida, the Florida SHOTS records may pre-populate to your account about one week after your UF acceptance.
How to view if your vaccine records have pre-populated:
- Log into the Student Self-Service Portal.
- Go to the Immunizations section.
- Click “View Missing Immunizations” to see if your vaccines are on file. If all are listed, you’re good to go!
- Check your One.UF account to confirm your immunization hold has been cleared.
Any missing vaccines will appear in your To-Do Items.
I am an out-of-state student:
If you are an out-of-state student, you may be able to request your vaccine records through your state’s health department.
How to request your vaccine records from your state:
- Log into the Student Self-Service Portal.
- Go to the Immunizations section and click the blue icon to request your records.
- Select the state(s) where you received vaccinations.
- Enter your name and associated address for each state.
- Click “Finish & Complete” and agree to the terms.
Check back on your Immunizations page after a few hours for updates.
I need to submit new records:
If your vaccines are not listed in a state registry, you’ll need to complete the UF Immunization Form and upload it manually.
Step 1: Prepare Your Documentation
- Have your doctor’s office, clinic, or health department fill out the required UF Immunization form.
- Ensure the record is signed, dated, and stamped by the health care provider or office representative.
- Any corrections must be initialed and dated by the provider.
- The document must be legible and in English (illegible records will not be processed).
- Keep a copy for your records.
Step 2: Enter and Upload Your Records
- Log into the Student Self-Service Portal and go to the Immunizations page.
- Click each dropdown menu to view available vaccine options.
- Select each vaccine that appears on your record and enter your vaccination dates.
- Repeat this process for all required vaccines.
- When finished, click “Submit.”
- Review the dates on the confirmation page.
- Upload your completed UF Immunization Form and any supporting lab reports in the same session. (Your dates will not be saved unless you upload your form before logging out.)
- A confirmation window will appear once your upload is successful.
Step 3: Follow Up
- Allow about one week for verification.
- Check your One.UF account to confirm your immunization hold has been cleared.
Any missing vaccines will appear under your To-Do Items.
Frequently Asked Questions
Who is required to show proof of immunizations?
All new University of Florida students, except certain distance learners, must provide proof of required immunizations before attending UF. This applies to anyone taking classes or participating in course activities on the main campus or at any UF location.
International students and most health-related majors must also complete tuberculosis (TB) screening to confirm they are free of TB.
Distance learners may qualify for a group exemption, but enrolling in online classes alone does not automatically waive immunization requirements. Check with your department for details.
Non-degree seeking students must also submit proof of immunizations.
All required vaccines are available at the UF Student Health Care Center (SHCC), which accepts most health insurance plans.
What forms are required?
Students can provide one of the records listed below:
- Completed General Education Immunization Form.
- An official state immunization record from a state registry.
- Official immunization records from another college or university.
- Military vaccine records.
Please note that our office will make final determination regarding whether alternative records can be used in lieu of the UF Mandatory Immunization Form.

What vaccines are required for Health Professions Programs?
Two MMR vaccines or a positive titer showing immunity.
Completed Hepatitis B series or a positive titer showing immunity.
One Meningitis ACWY vaccine on or after 16 years or a signed waiver declining the vaccine.
A TDAP vaccine within the last 10 years.
Two varicella vaccines or a positive titer showing immunity
Tuberculosis test *Refer to the Health Professions Immunization Form for specific requirements
Additional vaccines or screenings may be required after matriculation for
participation in clinical residency or rotations.
What vaccines are required for Veterinary Medicine Program?
Two MMR vaccines or a positive titer showing immunity
One Hepatitis B vaccine, a positive titer showing immunity, or a signed waiver declining the vaccine
One Meningitis ACWY vaccine on or after 16 years or a signed waiver declining the vaccine
TD and/or TDAP within the last 10 years.

Vaccine Waivers and Exemptions
Hepatitis B Vaccine Waiver
On-campus students are required to have at least one dose of Hepatitis B, present lab evidence of immunity, or sign the waiver to the decline the vaccine. These are the steps to decline, or waive, the vaccine:
- Read the CDC’s Vaccine Information Statement
- Check the box underneath the Hepatitis B vaccine section of the UF Immunization Form
- Sign and date the waiver (You must provide a legal signature to waive the vaccine. Please do not type your name.)

Meningitis ACWY Vaccine Waiver
On-campus students are required to have one dose of Meningitis ACWY at 16 years old or sign the waiver to decline the vaccine. These are the steps to decline, or waive, the vaccine:
- Read the CDC’s Vaccine Information Statement
- Check the box underneath the Meningitis ACWY vaccine section of the UF Immunization Form
- Sign and date the waiver (You must provide a legal signature to waive the vaccine. Please do not type your name.)

Medical Exemption
Temporary or permanent medical exemption to vaccination will be granted after receipt of documentation. If a vaccine is contraindicated, have your doctor provide a note on letterhead stating the reason that you cannot receive the vaccine.
Personal Tenets or Religious Exemption
Students must provide a copy of their K-12 immunization exemption record. They will then be asked to sign the UF Exemption Form to transfer the K-12 exemption to higher education.
If you’re having trouble…
Review your records carefully to make sure everything below has been completed correctly. These are the most common issues that delay or prevent processing:
- Dates entered: Did you enter all vaccine and/or health test dates in the portal before uploading your records?
- Documents uploaded: Did you upload supporting documentation (vaccine record, lab report, etc.) after entering your dates?
- MMR dates: Were both MMR doses given after 12 months of age? (Two doses are required.)
- Vaccine timing: Were all vaccines given with the required time intervals between doses?
- Meningococcal ACWY: Was your vaccine given in 2005 or later and on or after age 16?
- Provider information: Is your form signed, dated, and stamped by a licensed health care provider or clinic?
- Dates and signatures: Was the form signed after all listed vaccines or lab tests were completed?
- Lab reports: Did you include lab reports for any titers, TB screenings, or chest X-rays?
- TB test timing: Was your TB skin test read within 48–72 hours?
- Waiver forms: If you’re submitting a waiver, is it the official UF form (not one from another college or institution)?
